Model Emergency Transfer Plan for Victims of Domestic Violence, Dating Violence, Sexual Assault, or Stalking. Owners/Agents can use HUD-5381 as a model to develop their own Emergency Transfer Plan. Model form contains only general provisions and any adoption of this plan must contain all required elements.*

Certification of Domestic Violence, Dating Violence, Sexual Assault, or Stalking, and Alternate Documentation. Form should be provided to victims seeking VAWA protections. This, or other qualifying third-party documentation, can be returned for the victim to seek VAWA protections. Must be submitted within 14 days of the request from the housing provider.*

Emergency Transfer Request for Certain Victims of Domestic Violence, Dating Violence, Sexual Assault, or Stalking. By submitting this form, the victim seeking VAWA protections is requesting an emergency transfer and certifying that the requirements of eligibility have been met.*

Lease Addendum should be signed upon admission to the project. Any tenant in place prior to implementation of VAWA is also required to sign a Lease Addendum.*
