Two different certification forms are listed below. The first form is for public housing agencies (PHAs) completing their annual or five-year plans, and the second form is for all applicants who are seeking federal housing funds from an agency that requires the certification.

PHA Plan Certification Procedure
The U.S. Department for Housing and Urban Development (HUD) designed a new certification by State or local official of Public Housing Agency (PHA) plans consistent with the consolidated plan for all new PHA plans with an effective date of July 1, 2016, and later. This new form requires that Kentucky Housing Corporation (KHC) specify how the PHA plan is consistent with the consolidated plan, as well as the state's Analysis of Impediments to Fair Housing Choice (AI). Kentucky's AI is available on KHC's website, under Fair Housing in Kentucky.
In order for KHC to process the new PHA Plan certification, PHAs will be required to complete the Certification by PHA Official or PHA Plan Consistency pdf form below. Once completed, the authorized PHA official must sign the document and send it to KHC for review. The signed document should be scanned and attached to the Certification by State or Local Official of PHA Plans Consistency with the Consolidated Plan online request form.
