Please note that there are two different certification forms below. The first form is for public housing authorities (PHAs) completing their annual or five-year plans. The second form is for all applicants who are seeking federal housing funds from an agency that requires the certification.

The Kentucky Consolidated Annual Performance and Evaluation Report (CAPER) is prepared by Kentucky Housing Corporation (KHC) and the Department for Local Government and submitted to the U.S. Department of Housing and Urban Development Office of Community Planning and Development.  The CAPER details how the state utilized block grant funds for affordable housing during federal fiscal year 2008.

CDBG-R Substantial Amendment

The link below is to a Substantial Amendment to the Action Plan for Fiscal Year 2008 submitted by the Commonwealth of Kentucky.  The Action Plan is the annual update to the Consolidated Plan for FY 2004 through 2008.  The Commonwealth of Kentucky will receive $7,277,180 from the United States Department of Housing and Urban Development (HUD) through the American Recovery and Reinvestment Act of 2009  (ARRA).

The ARRA was signed into law by President Obama on February 17, 2009.  Title XII of Division A of the ARRA appropriates $1 billion nationwide to carry out the CDBG program (CDBG-R). It includes measures to modernize our nation's infrastructure, enhance energy independence, expand educational opportunities, and preserve and improve affordable health care. Unless otherwise stated, the grants are to be considered Community Development Block Grant (CDBG) funds and will be administered by the Department for Local Government (DLG).

Homelessness Prevention and Rapid Re-Housing Program (HPRP) Icon Adobe pdf 

The Homeless Prevention and Rapid Re-Housing Program (HPRP) is a HUD program funded under the American Recovery and Reinvestment Act (ARRA).  Kentucky Housing Corporation (KHC) has developed a substantial amendment to the 2008 annual Consolidated Action Plan .  The HPRP amendment to the Action Plan describes the state’s plan for distribution of federal housing dollars allocated by HUD under the ARRA.

Neighborhood Stabilization Program (NSP) funds were allocated to Kentucky through the Department for Local Government (DLG) from the federal Housing and Economic Recovery Act of 2008, necessitating an amendment to the existing Consolidated Action Plan. The amendment details the distribution method and allowable uses for this funding, which is a subset of Community Development Block Grant (CDBG) Funds administered by DLG.
