Any information collected through the Story Form will be used at the discretion of Kentucky Housing Corporation and its affiliates.

I authorize Kentucky Housing Corporation and its affiliates to use my/my client’s personal story about how I/my client was assisted with housing issues. I/my client understand(s) that this story and any supplemental materials, such as photographs, may be used in print, online, with the media, etc.

I understand that by completing this form my/my client’s story will be submitted to Kentucky Housing Corporation and its affiliates. I further understand that my/my client’s story could be used as an educational/discussion tool for the partners of said organizations, reporters, legislators, and may be excerpted in press coverage.

I understand that a pseudonym may be used to protect my/my client’s identity. Should I choose this option, I understand that it is my responsibility to inform KHC by choosing that option on the Story Form or by calling (800) 633-8896 or (502) 564-7630, ext. 454.

I understand that if I decide that I want to change how my/my client’s story is published, I should contact KHC immediately.

If I choose to submit a photograph, I authorize KHC, its representatives and employees, to use and publish the photo in print and/or electronically.

I agree that KHC may use such photographs of me with or without my name and for any lawful purpose, including as publicity, illustration, advertising, and Web content.

If I am submitting the Story Form on behalf of a client, I understand that the client must complete the Publicity Release Form before his/her story can be used in any way.
